Field Nurse

ExtraCare is seeking a nurse educator to work with patients and medical teams.

The core of the role is training patients and supporting them while they receive long-term treatment.

The role includes accompanying and training patients and medical staff, providing phone counseling to patients who need professional medical and emotional support, answering questions and solving problems, and monitoring their condition throughout treatment.

This is a field position combined with office work.

In addition, the nurse performs telephone health assessments using a structured questionnaire for initial evaluation, including documentation, follow-up and data reporting in a dedicated computerized system.

The role involves both independent work and teamwork, in coordination with relevant professional stakeholders.
